Evolving Motion Physical Therapy

EMPT is an out of network mobile clinic. This means we accept check, cash or credit card at the time of service and do not accept insurance at this time. We do provide a super bill which can be found on our interactive PTeverywhere software program following each treatment or can provided upon request. The super bill will have all the correct codes if you choose to submit claims to your insurance company for reimbursement. Please check with your insurance provider if you need authorization or physician orders prior to treatment. EMPT would be considered an Out of Network (OON) provider. EMPT is not responsible for your insurance reimbursement.

At this time I do not work with those would have Medicare as their primary insurance unless it is a wellness visit. Although, I would love to work with this population, I am not allowed to due to insurance rules.
